Resources for Health Professionals
The following documents were developed by HEARD Alliance participants because of concerns that adolescents being discharged from inpatient facilities may return to the community without the relevant community-based helping professionals being sufficiently aware of the hospitalization or the aftercare plan. The first document is, essentially, a procedure – and represents what the HEARD Alliance considers to be a “best practice.” The second document is a recommended form (“After-Visit Summary”) – potentially to share with the patient and parents/guardians at discharge – and to serve as a vehicle for communication between inpatient providers and community-based professionals.
